Per Diem Home Health Nurse Practitioner (NP) - Bogart, GA

Deliver impactful patient care from the comfort of patients homes through comprehensive risk assessments as a Nurse Practitioner specializing in Home Health. This per diem position offers flexibility and autonomy, allowing you to create your own schedule while serving a Medicare Advantage population. Home visits take place throughout Oglethorpe County and its surrounding communities, providing crucial annual health assessments and post-discharge follow-ups.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform home health risk assessments for Medicare Advantage plan members
• Review and document patient health history, current medical conditions, and medications
• Educate patients on their conditions and proper medication management
• Identify any gaps in care, offering recommendations to enhance patient outcomes
• No requirement for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Georgia
• Current CPR certification (BLS)
• Board Certification via AANP or ANCC
• Minimum six months recent experience as a Nurse Practitioner preferred
• MSN degree from an accredited nursing program

Compensation for this position is structured at $80$90 per evaluation, with additional stipends for mileage and administrative time, acknowledging the importance and value of your expertise in each patient visit.
