Deliver essential care in a vital home-based setting as a Nurse Practitioner providing risk assessments for Medicare Advantage members throughout Oglethorpe County. This per diem opportunity affords both professional autonomy and flexible scheduling, suited for experienced nurse practitioners who thrive in community-based care.

Schedule:

• Flexible shifts offered between Monday and Sunday
• Set your own schedule
• Commit to a minimum number of hours on a weekly or bi-weekly basis

Key Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage plan members in locations such as Lexington, Crawford, and Maxeys
• Review and evaluate prior and current medical history, present conditions, and medication usage
• Deliver education to patients regarding their medical conditions and medications, supporting their understanding and self-management
• Identify any gaps in care and offer actionable recommendations to support improved health outcomes
• Documentation of findings is required; however, treatment plan creation and prescribing are not

Qualifications:

• Active Nurse Practitioner license in Georgia
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• At least 6 months of current NP experience (preferred)
• MSN from an accredited nursing program

Compensation ranges from $80$90 per completed evaluation, and a mileage stipend plus administrative time is included.
