Per Diem Home Health Nurse Practitioner in Midway, GA

A skilled Nurse Practitioner is needed to provide per diem home health risk assessments with a focus on exceptional patient care. This role offers the flexibility to structure your schedule with shifts available Monday through Sunday and allows practitioners to select availability that works best for their routines. Work takes place within Liberty County, including Hinesville, Midway, and Riceboro, and assignments will be made in any of these areas during individual shifts.

Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Assess prior and current medical history, conditions, and medication records
• Provide clear education to patients about their medical issues and prescribed medications
• Identify gaps in care and propose actionable recommendations to enhance patient outcomes
• Focus strictly on evaluationsno requirement to prescribe medications or develop treatment plans

Desired Qualifications:

• Valid Nurse Practitioner license in Georgia
• Current CPR (BLS) certification
• Board certification through AANP or ANCC
• 6 months of recent nurse practitioner experience is preferred
• MSN degree awarded by an accredited nursing school

Compensation is based on $80$90 per completed evaluation, with additional mileage and administrative time stipends, offering both flexibility and recognition of your expertise.
