Take the lead in delivering quality home health care as a Nurse Practitioner specializing in risk assessments for Medicare Advantage plan members. This per diem opportunity allows you to conduct comprehensive home visits throughout Chatham County, including Savannah, Pooler, and Port Wentworth, ensuring flexibility and work-life balance.

Schedule:

• Flexible shifts between Monday and Sunday
• Customize your own work hours
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members in various settings
• Review and evaluate prior and current medical history, medical conditions, and medications
• Educate patients on their health conditions and medication regimens
• Identify gaps in care and recommend appropriate next steps to promote patient well-being
• Focus solely on assessment and educationno need to create treatment plans or prescribe medication

Qualifications:

• Active Nurse Practitioner license in Georgia
• Board Certification through AANP or ANCC required
• Current CPR certification (BLS)
• MSN degree from an accredited nursing school
• At least 6 months of recent nurse practitioner experience preferred

Compensation: Each completed evaluation is compensated at a competitive rate of $80 to $90, in addition to a mileage stipend and administrative time, recognizing the importance of your expertise and time.
