Per Diem Nurse Practitioner Home Health Cedar Rapids, Iowa

Step into a meaningful per diem opportunity as a Nurse Practitioner specializing in Home Health risk assessments for Medicare Advantage plan members throughout Linn County. Providers will deliver care in a variety of community settings, including Cedar Rapids, Marion, and Hiawatha. This role offers a great degree of autonomy and varietyconducting thorough home health risk assessments, with each visit averaging about 45 minutes. The nature of each visit may vary, from annual health evaluations to post-discharge assessments.

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage plan enrollees
• Review prior and current medical history, ongoing conditions, and medication usage
• Educate patients regarding their health conditions and medications
• Detect gaps in care and recommend solutions to promote optimal health outcomes
• Note: No requirement for treatment planning or prescribing medications

Desired Qualifications:

• Active Nurse Practitioner license in Iowa
• Current CPR certification (BLS)
• Board Certification via AANP or ANCC
• Minimum of 6 months current NP experience (preferred)
• MSN from an accredited nursing program
