Per Diem Home Health Nurse Practitioner in Marion, IA

As a skilled Nurse Practitioner with a passion for patient-centered care, this per diem opportunity allows you to make a direct impact through home health risk assessments for Medicare Advantage health plan members across Linn County, including Cedar Rapids, Marion, and Hiawatha. Schedule:

• Flexible shifts available Monday through Sunday
• Set your own hours
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage members during home visits
• Evaluate medical history, ongoing medical conditions, and medication use
• Educate members on their health, diagnoses, and medication regimens
• Identify care gaps and provide actionable recommendations to close them
• No responsibility for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Iowa
• Current BLS/CPR certification
• Board Certification through AANP or ANCC
• At least six months of recent Nurse Practitioner experience preferred
• Masters degree in Nursing (MSN) from an accredited school

Youll earn between $80 and $90 per risk assessment evaluation, with additional compensation for mileage and administrative work.
