Key Responsibilities:

• Carry out comprehensive home health risk assessments for Medicare Advantage plan members
• Assess patients prior and current medical history, ongoing conditions, and medication history during each evaluation
• Deliver clear education to patients regarding their medical conditions and medications
• Identify care gaps and communicate actionable recommendations to improve patient health outcomes
• Focus exclusively on assessmentsno prescribing or development of treatment plans required

Qualifications:

• Current Nurse Practitioner license in Ohio
• Valid CPR certification (BLS)
• Board Certification via AANP or ANCC
• At least 6 months of recent NP experience preferred
• MSN degree from an accredited program
