Per Diem Home Health Nurse Practitioner in Ottumwa, IA

Schedule:

• Flexible shifts between Monday and Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

What You'll Do:

• Conduct detailed in-home risk assessments for Medicare Advantage health plan members
• Review past and current health histories, medical conditions, and medication regimens
• Provide clear education to patients regarding their conditions and medications
• Identify care gaps and make actionable recommendations to improve patient outcomes
• Complete assessments in approximately 45 minutes per visit

Desired Experience & Qualifications:

• Active Nurse Practitioner license in IA
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Graduate of an accredited MSN program
• At least 6 months of Nurse Practitioner experience preferred
