Per Diem Home Health Nurse Practitioner (NP) - Montrose, Iowa

Key Responsibilities:

• Perform comprehensive annual risk assessments for Medicare Advantage plan members, focusing on a detailed review of current and past medical history, existing medical conditions, and medications
• Conduct each evaluation efficientlymost visits require approximately 45 minuteswith location assignments across Keokuk, Fort Madison, Montrose, and surrounding areas in Lee County
• Educate patients about their health, medications, and best practices for ongoing care
• Identify potential gaps in care and provide evidence-based recommendations to bridge them
• Please note: no treatment planning or prescription writing is involved

Required Qualifications:

• Active Nurse Practitioner license in Iowa
• Board certification through AANP or ANCC
• Current CPR certification (BLS required)
• MSN degree from an accredited nursing program
• At least six months of recent nurse practitioner experience preferred
