Each assessment averages 45 minutes and may include annual health reviews or post-discharge evaluations, allowing you to provide meaningful one-on-one care outside of a traditional healthcare setting.

Responsibilities:

• Deliver comprehensive annual home health risk assessments for Medicare Advantage members
• Analyze and review patients medical histories, current conditions, and medication profiles
• Educate members on health conditions and effective medication usage
• Detect gaps in care and offer actionable resolutions to improve patient outcomes
• Note: This position does not require developing treatment plans or prescribing medication

Qualifications:

• Active IA Nurse Practitioner license
• Board certification via AANP or ANCC
• Current CPR (BLS) certification
• MSN from an accredited nursing program
• At least 6 months of recent NP experience is preferred
