Per Diem Home Health Nurse Practitioner in Fort Madison, IA

As a Nurse Practitioner specializing in home health, youll be responsible for conducting risk assessments for Medicare Advantage plan members located throughout Lee County, including Keokuk, Fort Madison, and Montrose. This role offers the chance to independently schedule visits based on your availability while ensuring comprehensive evaluations and support for patients transitioning through critical stages of care.

Schedule:

• Flexible shifts available seven days a week
• Design your own work schedule
• Commitment required on a minimum weekly or bi-weekly basis

Key Responsibilities:

• Conduct thorough annual risk assessments in patients homes, evaluating medical history, conditions, and medication use
• Educate patients about their health conditions and prescribed medications to promote understanding and adherence
• Identify potential gaps in care and suggest recommendations to ensure the best possible patient outcomes
• Complete documentation without responsibility for developing treatment plans or prescribing medications
• Each evaluation is expected to take about 45 minutes

Qualifications:

• Active Nurse Practitioner license in Iowa
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• At least 6 months of recent nurse practitioner experience preferred
• MSN degree from an accredited nursing program

Youll receive $80-$90 for every risk assessment completed, along with a mileage stipend and compensation for administrative time.
