Per Diem Home Health Nurse Practitioner in Athens, GA

Enjoy the autonomy and flexibility of a per diem opportunity as a Nurse Practitioner specializing in home health. Youll have the chance to create your own schedule with shifts available any day of the week, Monday through Sunday. This role involves conducting comprehensive home health risk assessments for Medicare Advantage members throughout Oglethorpe County, including Lexington, Crawford, and Maxeys. Each home visit typically averages 45 minutes and may involve annual health assessments or post-discharge evaluations, allowing a meaningful impact on members health and quality of care without the responsibility of prescribing or treatment plan creation.

Schedule:

• Flexible shiftswork any day Monday through Sunday
• Create your own schedule to fit your availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct in-home risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, conditions, and medications
• Educate members on related medical topics and proper medication usage
• Identify care gaps and provide recommendations for ongoing care
• No need for prescribing medications or establishing treatment plans

Qualifications:

• Active Nurse Practitioner license in Georgia
• Board Certified through AANP or ANCC
• Current CPR (BLS) certification
• MSN from an accredited program
• At least 6 months of current NP experience preferred

Compensation and Benefits:
