Per Diem Nurse Practitioner Home Health Dublin, Georgia

In this per diem position, you'll play a crucial role in improving patient outcomes by conducting comprehensive health evaluations for Medicare Advantage plan members living in Dublin and throughout Laurens County, GA. Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform in-home annual health risk assessments for Medicare Advantage members
• Review prior and current medical history, health conditions, and medications
• Educate patients on chronic medical conditions and medication adherence
• Identify gaps in care and offer actionable recommendations for follow-up
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Valid Nurse Practitioner license in Georgia
• Board certification from AANP or ANCC
• MSN (Master of Science in Nursing) from an accredited institution
• Current CPR certification (BLS)
• At least 6 months of Nurse Practitioner experience preferred
