Per Diem Home Health Nurse Practitioner (NP) - Wrightsville, Georgia

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Opportunity for minimum weekly or bi-weekly hourly commitments

Responsibilities:

• Perform comprehensive home health risk assessments, averaging 45 minutes per visit
• Assess prior and current medical history, existing medical conditions, and medication use
• Deliver patient education regarding diagnoses and medications
• Identify care gaps and provide actionable recommendations for improved health outcomes
• Focus strictly on assessment and educationno responsibility for prescribing or treatment planning

Qualifications:

• Active Nurse Practitioner license in Georgia
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of recent nurse practitioner experience preferred
