Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimum required commitment of hours weekly or bi-weekly

Responsibilities:

• Manage annual home health risk assessments for Medicare Advantage plan members
• Review and document prior/current medical history, conditions, and medication use
• Deliver patient education regarding conditions and medication management
• Identify potential gaps in care, offering recommendations for improvement
• Focus on assessment and educationno treatment planning or prescribing needed

Qualifications:

• Active Nurse Practitioner license in Georgia
• MSN degree from an accredited nursing program
• Current CPR (BLS) certification
• Board Certification (AANP or ANCC) required
• At least 6 months of recent Nurse Practitioner experience preferred
