Schedule & Work Structure:

• Flexible shifts available Monday through Sunday

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members
• Review prior and current medical history, assess medical conditions, and evaluate medication usage
• Educate patients on their health conditions, medications, and preventive care
• Identify and provide recommendations for addressing care gaps
• No requirement for treatment plan development or medication prescriptions

Qualifications:

• Active Nurse Practitioner license in Georgia
• Board certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN degree from an accredited nursing school
• At least 6 months of current nurse practitioner experience preferred
