Key Responsibilities:

• Perform comprehensive annual risk assessments and post-discharge visits for Medicare Advantage members
• Evaluate prior and current medical history, existing medical conditions, and medication use
• Educate patients about chronic conditions, healthy living, and safe medication practices
• Identify and document gaps in care, offering recommendations for resolution
• No treatment plan development or medication prescribing required

Qualifications:

• Current Georgia Nurse Practitioner license
• Active CPR certification (BLS)
• National Board Certification (AANP or ANCC)
• At least 6 months of recent NP experience preferred
• Master of Science in Nursing (MSN) from an accredited program
