Per Diem Home Health Nurse Practitioner (NP) - Bloomingdale, GA

This per diem opportunity offers the flexibility to create your own schedule, with shifts available any day between Monday and Sunday, and a minimum weekly or bi-weekly hourly commitment to fit your availability.

Schedule:

• Flexible shifts Monday through Sunday
• Design your own schedule
• Minimum hourly commitment required weekly or bi-weekly

Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
• Evaluate prior and current medical history, medical conditions, and medication histories
• Provide patient education on medical conditions and medications
• Identify gaps in care and make recommendations to close those gaps
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Georgia (GA)
• Current BLS CPR certification
• Board Certification through AANP or ANCC
• At least 6 months of current Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) degree from an accredited nursing school
