Deliver vital care directly to Medicare Advantage plan members through per diem home health risk assessments. As a Nurse Practitioner focusing on home health, youll visit patients in Des Moines County, IA, covering areas such as Des Moines, Cedar Rapids, and Davenport. The role consists of conducting thorough risk assessments, each taking approximately 45 minutes, within patients homes. Each encounter may involve annual health reviews or post-discharge evaluations, providing an essential layer of support for members' ongoing health needs.

Responsibilities:

• Complete home-based risk assessments for Medicare Advantage members
• Review medical history, current medical conditions, and medications
• Provide patient education regarding diagnoses and therapies
• Identify care gaps and suggest potential solutions to help bridge them
• No requirement to develop treatment plans or prescribe medication

Qualifications:

• Active Nurse Practitioner License in Iowa
• Current CPR certification (BLS)
• Board certification via AANP or ANCC
• At least 6 months of experience as a Nurse Practitioner preferred
• MSN degree from an accredited nursing program
