Each evaluation typically takes about 45 minutes and may include annual health assessments or post-discharge visits. Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, medical conditions, and medication usage
• Educate patients regarding their health conditions and medications
• Recognize and report care gaps, offering recommendations for improvement
• Focus on evaluationtreatment planning and prescribing are not required

Qualifications:

• Current Iowa Nurse Practitioner License
• Active CPR certification (BLS)
• Board Certification through AANP or ANCC required
• Minimum of 6 months recent Nurse Practitioner experience preferred
• MSN degree from an accredited nursing program
