Youll be performing home health risk assessments for Medicare Advantage members, visiting individuals in their residences throughout Polk County, IAincluding Urbandale, Johnston, and Clive. Assessments average 45 minutes and may involve annual wellness checks or post-discharge follow-ups.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: create your own work calendar
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete home health risk assessments, documenting findings for a Medicare Advantage health plan
• Assess previous and current medical history, current conditions, and medication use
• Offer patient education on health conditions and medication regimens
• Identify gaps in care and propose actionable recommendations
• Focus exclusively on evaluation; no prescribing or treatment plan development required

Qualifications:

• Active Nurse Practitioner license in Iowa
• CPR certification (BLS)
• Board Certification (AANP or ANCC)
• At least 6 months experience as an NP preferred
