Per Diem Home Health Nurse Practitioner in Eddyville, IA

Embark on a dynamic per diem opportunity as a Nurse Practitioner specializing in home health assessments. This role revolves around conducting thorough risk evaluations for Medicare Advantage plan members residing throughout Wapello County, IA, in areas such as Ottumwa, Eldon, and Agency. The primary responsibility is to visit patients in their homes, dedicating approximately 45 minutes to each assessment, which may include annual health reviews or post-discharge evaluations. With a focus on clinical excellence, you will play a crucial part in promoting wellness and identifying opportunities for improved patient care.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Self-scheduling to suit your preferences
• Minimum weekly or bi-weekly hourly commitment required

Primary Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Review and evaluate patients prior and current medical history, including medical conditions and medication regimens
• Educate patients about their health conditions, preventive care, and proper medication use
• Identify any gaps in care and offer recommendations to improve patient outcomes
• This role does not require creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner License in IA
• Current CPR certification (BLS)
• Board Certification via AANP or ANCC
• At least 6 months of recent experience as a Nurse Practitioner strongly preferred
• Master of Science in Nursing (MSN) from an accredited institution
