Per Diem Home Health Nurse Practitioner (NP) - Pooler, GA

Deliver comprehensive care in a flexible per diem role as a Nurse Practitioner performing home health risk assessments across Chatham County. In this position, you will visit Medicare Advantage plan members in their homesincluding areas such as Savannah, Pooler, and Port Wentworthto conduct thorough, patient-centered risk assessments. Each visit takes approximately 45 minutes and may include annual health assessments or post-discharge evaluations. Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Minimum weekly or bi-weekly commitment required

Primary Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members
• Review prior and current medical history, conditions, and medication usage
• Educate patients about their medical conditions and prescribed medications
• Identify and address gaps in care, recommending appropriate solutions
• No requirement for developing treatment plans or prescribing medications

Desired Qualifications:

• Nurse Practitioner License (GA)
• Current BLS/CPR certification
• Board Certification (AANP or ANCC)
• 6+ months of experience as an NP preferred
• MSN degree from an accredited nursing program
