Per Diem Home Health Nurse Practitioner (NP) - Waukee, Iowa

Schedule:

• Flexible shifts offered every day of the week, Monday through Sunday

• Minimum hourly commitments set weekly or bi-weekly

Key Responsibilities:

• Conduct thorough annual risk assessments for Medicare Advantage members residing in Polk County
• Complete evaluations in various communities, including Urbandale, Johnston, and Clive, depending on patient locations for each shift
• Assess and document previous and current medical history, medical conditions, and medication regimens
• Educate patients about their health conditions and prescribed medications, providing information that empowers better self-care
• Identify care gaps and make recommendations for further action, aiming to optimize each patients health journey

Qualifications:

• Active Nurse Practitioner license in the state of Iowa
• Current CPR certification (BLS required)
• Board Certification through the AANP or ANCC
• Minimum of six months' experience working as a Nurse Practitioner preferred
• MSN from an accredited nursing program

This role offers a competitive pay structure, with compensation of $80$90 per evaluation. Additional reimbursements include a mileage stipend for travel and compensation for administrative time.
