As a Nurse Practitioner, youll connect with Medicare Advantage members during in-home visits, focusing on annual assessments and post-discharge check-ins. Each evaluation averages approximately 45 minutes, allowing you to provide personalized attention while maintaining a flexible lifestyle. Youll be scheduled for visits in various communities throughout Lee County, including Keokuk, Fort Madison, and Montrose, with shift locations varying based on scheduling needs.

Schedule:

• Flexible shifts available seven days a week
• Choose your own schedule
• Minimal weekly or bi-weekly hourly commitments

What Youll Do:

• Complete thorough annual home health risk assessments for Medicare Advantage plan members
• Assess medical history, conditions, and current medications
• Offer patient education regarding health conditions and prescribed medications
• Recognize and document any care gaps, providing recommendations for improvement
• Focus exclusively on assessmentsno treatment plan development or medication prescribing required

Qualifications:

• Active Nurse Practitioner License in the state of IA
• Current BLS/CPR certification
• Board Certified through AANP or ANCC
• Six months or more of recent Nurse Practitioner experience preferred
• MSN from an accredited nursing program

You will receive competitive compensation of $80-$90 per evaluation, along with a mileage stipend and pay for administrative time.
