Per Diem Home Health Nurse Practitioner in Eldon, IA

Deliver expert care and make a difference in your local community by performing home health risk assessments for Medicare Advantage plan members. This per diem opportunity is designed for Nurse Practitioners seeking flexible scheduling and the ability to set their own hours. You will conduct comprehensive home visits within Wapello County, which includes areas such as Ottumwa, Eldon, and Agency. Each visit typically takes about 45 minutes and may range from annual health assessments to post-discharge evaluations.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule to suit your availability
• Minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

• Perform annual risk assessments and comprehensive health evaluations in members homes
• Review and evaluate both prior and current medical histories, including medications and existing conditions
• Educate members on their medical conditions and medication regimens
• Identify gaps in care and deliver actionable recommendations to support care continuity
• Note: No need to create or manage treatment plans, nor prescribe medications

Qualifications:

• Nurse Practitioner license in the State of Iowa
• Active Board Certification through the AANP or ANCC
• Current CPR certification (BLS)
• Minimum of 6 months recent NP experience preferred
• MSN from an accredited school of nursing
