Per Diem Home Health Nurse Practitioner in Ankeny, IA

Take on a vital role as a Nurse Practitioner providing high-quality home health risk assessments for Medicare Advantage plan members across Des Moines County, IA. This per diem opportunity offers remarkable flexibility, allowing you to design your own schedule with shifts available any day of the week, Monday through Sunday. Candidates may be assigned to locations within Des Moines, Cedar Rapids, Davenport, and other areas in Des Moines County during each shift.

Schedule:

• Flexible shifts available throughout the week, Monday to Sunday
• Ability to create your own schedule
• Meet minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments, typically completed in approximately 45 minutes per visit
• Obtain and evaluate comprehensive prior and current medical history, including medical conditions and medication usage
• Provide patient-centered education on diagnoses, conditions, and prescribed medications
• Identify any care gaps and recommend solutions to help support continuity of care
• No obligation to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Iowa
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• Minimum of 6 months experience as an NP preferred

This role offers a competitive per-evaluation payment of $80$90, with a mileage stipend and compensation for administrative time.
