Per Diem Home Health Nurse Practitioner (NP) - Des Moines, Iowa

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling for maximum work-life balance
• Minimum commitment options, either weekly or bi-weekly

Key Responsibilities:

• Complete comprehensive annual or post-discharge home health risk assessments
• Review and evaluate patients medical history, existing conditions, and medication regimens
• Educate members on their health conditions and prescribed medications
• Identify care gaps and offer actionable recommendations to help close them
• No requirement for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Iowa
• Board Certification through AANP or ANCC
• Current BLS (CPR) certification
• MSN from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience strongly preferred
